I don't recall the name of it, but isn't there an add-on that enables you to disable/enable favorites?
Maybe instead of deleting them and having to add them back later someone having this trouble could see if just disabling them with this add-on would have the same effect as deleting the offending favorite. It would make it much easier than manually deleting/re-adding favorites.

Of course a definitive answer on why it is happening would also be great.

Also, to those of you who have gone the route of starting with a fresh wiz.bin and that working for a while and then the problem reappearing...... I still posit that there is a bad recording somewhere in your system. When SageTV first starts with a new wiz.bin everything is fine, but eventually it screws up. Now that could be just when SageTV does a rescan of your library, or maybe when you actually try to play that video.

I would suggest leaving the wiz.bin alone (I am still using a wiz.bin that started back in like version 4 of SageTV), shut down SageTV/Service and renaming your recordings folders and recreating new folders in the same path(s) with the original name. This would definitely get any offending recording out of SageTV's view.
Then see how long SageTV works properly. If it continues to work then you could start merging some of the old recordings back into the *new* folders a few at a time until you find the offending file(s).
I would think that files older than a week or so from the time the problem first cropped up should be fine to add back en-mass. Then add back the rest a few at a time, wait and see if the problem returns.

I know I sound like an old drum here, but on at least 3 occasions in the past this exact same thing has happened to me, and each time it was due to a bum recording. At least one of those played fine, but would kill the scheduling of recordings.

When I was trouble shooting, I moved all my recordings out to a TEMP folder so the original was empty. It stayed broken.

I think this is a Guide data issue. One thing that changed recently was the addition of new channels to my lineup. Most of them for odd sports and Spanish language. I have disabled all these extral channels to see if that does the trick. So far most of our favorites have been added back and scheduled recordings still look good.

Somewhere in the options, you can turn Automatic Add of New Channels to OFF

I have it that way so my guide doesn't get cluttered with channels I care nothing about. Still get the System Messages when new Channels are available and then I just Enable those that I care about

As graywolf suggested, turn off automatic adding of new channels. You will still get a notification that there are new channels in the message area of SageTv. I turned mine off ages ago, mostly because with different tuners getting different channels automatic add was giving me fits.

Also, there may just be an EPG issue. I just had a conflict and went to set a later airing as Manual to free up the "conflicting" spot at 8pm. There was another airing later and even though its data said it was a first run it wasn't showing as one of the future airings of my favorite.
This was an isolated issue I have seen very rarely in the past, but perhaps on your lineup whatever data glitch is in there is much more widespread.

I have also seen issues that can mess with favorites whenever my guide data had "holes" in it with sections of no data on certain channels.
