If I bought my own 211k, would I have to pay any "additional receiver" fees, or is that just if Dish provides the hardware?

Yup there is no bonus to purchasing aside from R5000HD, otherwise what would you do with a purchased box without the service. There are tuner limits once you pass those you have no choice but to purchase them.
EDIT: Also be damn careful if you are buying used boxes, most are leased and hence cannot be sold. Others if the previous owner has a balance on the account you can't activate it until that is paid.
