OTA Channels disappear when add new tuner

I have two tuners. One uses OTA antenna and one extended digital cable. When I set up the OTA card it finds all of the correct channels and lists them in the EPG. I then set up the cable card with the Brighthouse digital lineup and half the OTA channels disappear from the EPG. Not just data. The complete channels are gone, but only half of them.

Remove the digital cable source and all OTA channels are back in EPG. If I install the cable card as regular extended cable then all the OTA and cable channels show up in EPG, but only to channel 99. Set it up as digital cable and half the OTA channels disappear again. So now I have a choice of half of the OTA channels and channels above 99 in the EPG, or all OTA and nothing above 99. Anyone have any idea what would cause this?

I have removed all sources and started over with a Detailed Setup but no change. The EPG that seems to be removing the OTA is Brighthouse Volusia/Flagler Digital (extended) zip 32114. Thanks!
