QAM Recording - Hiccups, Errors, and Odd Behavior

I just don't know what to make of this corrupted recording (below).

Is it possible the cable company screwed up the feed so badly?

Or does it indicate some know problem with this type of setup?

Recording:

- File: Reba-ProudReba-2715551-0.mpg
- Time: 4:30 to 5:00
- Format: MPEG2-PS[MPEG2-Video 16:9 720p@60fps, AC3/192Kbps@48kHz Stereo eng]
- Recording File Size: 1.84 GB

Problems:

- SageTV UI reports Halt Detected in Recording 4:32 PM
- SageTV UI reports Halt Detected in Recording (x3) 4:33 PM
- During playback, file data is always 16:9.
- However, show content switches between HD and letterboxed SD several times.
- Some parts of the show are missing/corrupted.

Troubleshooting:

- I tried watching live TV and switching channels -- no apparent problems.
- ???

Configuration:

- SageTV 6.6.2
- Installed in C:\PVR to avoid Windows 7 UAC "feature"
- Kept Wiz.bin file (for recording history, favorites, etc.)
- Deleted all config files, including Sage.properties and client properties.
- Configured SageTV to run as service.
- Ran SageTV UI and did basic video configuration incl. auto load at startup.
- Closed SageTV UI and service; configured startup delay to 30 seconds so that SageTV doesn't randomly "forget" that it's supposed to run in service mode.
- Restarted computer.
- Configured ATI HD 650 analog source "TV Tuner"
- Configured ATI HD 650 digital source "Digital TV Tuner"
- Configured ATI HD 5770 for EVR w/ default MPEG-2 decoders
- Everything tested and worked fine incl. auto switching sources as dictated by channel changing.

My system:

- Windows 7 x64
- Intel E8400
- 8 GB RAM
- ATI HD 5770 1 GB (Catalyst 9.12)
- ATI HD 650 Combo PCI-E (Theater 9.12) w/ cable split to both jacks
- Realtek HD Audio
- separate HDDs for OS, data, and recordings
