Does this happen to all recordings i manually set? When does the "watched" status take hold exactly? Do I have to watch a show all the way through to get that status? Should I mark every show I watch?

It happens to any show any recording type even "live", which of course is recorded first for timeshifting. Shows are set to "watched" after you watch them OR set them to watched via the menu. "Should" you set them to watched? Personal choice. Setting a show to watched has the effect of not being recorded again via IR or Favorites, you can always Manually setup a show to record whether you've previously watched it or not.

see the reply I gave you yesterday:

if more than a certain percentage of the airing has been watched (I guess ~80%), then it will automatically be marked as watched when playback is stopped, otherwise it will still be not watched. You can see this just by stopping playback, then going to the detailed info screen.

If you never set a show to Watched, Intelligent recording has no idea of what type of show you like (Watched is equivalent to thumbsup).
However as mdmint mentioned, episodes marked as Watched will never be recorded ever again (if the TV data is good and each episode has its own unique ID).
