One 250-16 system, cheapest additional tuner?

Can I add one of these PVR-150 MCE for $60 or do I need to hunt down a $100 PVR-250-16? I don't want a headache, so I don't want to experiment

you could go with a pvr 250
but the newest pvr 250s were coming with a crappy tuner
the philips tuners were not used in the last few made
and many people have trouble with them even after the upgraded drivers to fix the non tuning issue

I would consider one of the pvr 150s
if you get a retail one
they come with an IR blaster

you might consider reading some posts at shspvr.com/forum
he has a section dedicated to the pvr 150

Do I care about the IR blaster if I already have my MX500 controlling sage with the IR port in the 250? I assume that adding a tuner does not change anything with respect to controlling Sage. So could I get by with an OEM or MCE?

Those forums don't have any post in the 150 sections yet, I guess it's too new.

Good question. I was thinking about getting a 150 also, but they were closer to $70 when I priced them.
