If I bought a RS232 linkable IR can I go direct to controlling the sagetv HDextender?
No, you'll need CQC. Although methinks you're making too much about a "production" CQC setup. Just install it on a given box, and only setup the IRA and Sage drivers, and you're done. No UI needed, it can be fully a "back-end" setup.

Has anyone been brave enough to attempt desoldering the IR receiver and extending it with wires?

I haven't cracked the case, so I don't have a clue about how it is connected, but it's tempting to try. It will be more tempting to try once the warranty period is over.

I would like to do this as well (maybe once the warranty period is over), but only to place the IR receiver on top of my flat screen and then tuck the box behind the display.

The other option I would like is to use the USB ports on the HD100 to plug in an IR receiver (like the ones that come with some tuner cards) but I haven't heard or seen anything on this.
